Te Whanau A Iwi Community Health Centre
In 2003, Te Whanau A Iwi Health Centre opened it’s doors at the Lytton West shopping centre. The centre has an enrolled population of 1,400 patients. This clinic has 2 part time doctors, 1 nurse and 1 receptionist. All employees are salaried. Twai has 3 consultation rooms and an administrative area. After hours cover is shared with the GP’s at Puhi Kaiti.
The staff at both clinics (Puhi Kaiti and Te Whanau A Iwi) are supportive of each other and weekly staff and team meetings are held to discuss issues.
To improve and efficiently manage the health of our people, Chronic Care nurses have been employed as a relatively new initiative to assist GP’s and Practise nurses to educate and assist those patients diagnosed with Chronic Diseases such as Heart Disease and Diabetes. This initiative has proved to be very successful and we are seeing positive results among those patients who access this service. We are determined to offer the best possible service to our patients. We have found the cost of healthcare to be a barrier for those families on low incomes and therefore keep our costs low so they are able to access Health services regularly. We are fortunate to also be able to offer patients access directly to services within Ngati Porou Hauora.
